CREDIT CARD AUTHORIZATION FORM

Please fill out the Credit Card Authorization form. You may choose to do so with each payment, or you may initial to allow us to keep your card on file for future payments for the current trip that you are booking. This is REQUIRED before we can process your deposit and any future payments for your account. 
Please print, sign, and return to your agent.

TRAVEL INSURANCE WAIVER
Please fill out this document if you are declining the Agent provided Travel Insurance. This is REQUIRED if you do not plan to purchase insurance through the Agency.

MINOR TRAVEL CONSENT

If you are traveling with a child who is not your own, or if you are the non-custodial parent of a child, you must fill out this form prior to traveling with Hat's Off Adventures. Please reach out to your Travel Agent with any questions.
